CARE Direct

CARE Direct is an innovative participant guided approach to self-monitored blood pressure management. In collaboration with the CARE Collaborative, CARE Direct focuses on those who are 18 years and older who are experiencing pre-hypertension, hypertension and/or as a preventive measure.

Each participant will meet with a program facilitator for at least 30 minutes once a month for 6 months. Guided through the program with motivational interviewing, the participants will decide which educational topic to discuss and learn about while also discussing the proper techniques to take blood pressure and use a blood pressure monitor. We found by having the participant determine how the educational sessions will go, they will keep remain engaged and wanting to continue to attend until the program is complete on their journey to improving their blood pressure and overall health.
