Stroke Systems of Care
The Stroke Systems of Care committee exists within the Kentucky Heart Disease and Stroke Prevention Task Force to create a simplified resource library for use within any healthcare organization or community outreach program. The committee has identified thirteen priority topics related to heart disease and stroke in Kentucky. Each of these topics has specific resources that have been reviewed by the committee and chosen on the basis of evidence-based education. These resources exist to standardize and reinforce the information patients receive from various providers. These resources are available below as downloadable PDFs to be printed at your own convenience.
