Our Mission
The mission of SEQIP is to advance acute stroke care management and reduce stroke disparities in Kentucky by:
- Establishing a network of professionals that will encourage and support collaboration among stroke care providers.
- Providing opportunities to share information and resources related to stroke program development and proficiency across the continuum of care.
- Promoting quality, improving outcomes and standardization of acute stroke care through collegiality and use of evidence-based practice guidelines.
About Us
SEQIP was created in 2009 as a statewide voluntary stroke quality improvement initiative of the Kentucky Heart Disease and Stroke Prevention Task Force – Cardiovascular Health (CVH) Delivery Systems Subcommittee and the American Heart Association/American Stroke Association (AHA/ASA). SEQIP initiated voluntary participation in a stroke registry for hospitals, the first in Kentucky. SEQIP has grown from 16 participating hospitals in 2009 to over 40 hospitals in 2022.
Since its inception, SEQIP has grown to include representatives from all certified stroke centers in the state, as well as hospitals seeking certification and other individuals and organizations with an interest in improving stroke care. Participation in SEQIP is voluntary and open to all hospitals and stakeholders in Kentucky and surrounding areas.
In 2012, Kentucky Statute KRS 211.575 became law, and enactment in the 2022 legislative session requires all certified stroke centers to report to the registry each case of stroke seen in their facility. Then annually, the Kentucky Department for Public Health provides a report of SEQIPs data and any related findings and recommendations to the Governor and to the Legislative Research Commission.
Participation
Participation in SEQIP is voluntary and open to all hospitals and stakeholders in Kentucky and surrounding areas. Membership does not require registry participation. Membership is free. If you are interested in improving stroke systems of care in The Commonwealth and surrounding states, we would love to have you as a member.

Kari Moore is an acute care nurse practitioner who specializes in stroke and cerebrovascular disease with over 20 years of experience. She is currently the Executive Director for Patient Care Services at Regional Brain Institute which partners with health systems in six states across the Midwest. Kari serves on the Kentucky Heart Disease and Stroke Prevention Task Force Steering Committee, is a member of the Board for the Association of Neurovascular Clinicians, co-chairs the Stroke Encounter Quality Improvement Project focused on improving statewide stroke systems of care and serves as a co-investigator for the Kentucky Paul Coverdell National Acute Stroke Program funded by the Centers for Disease Control and Prevention. Her passion is bringing stakeholders together across geopolitical boundaries to improve access to care and reduce disparities. Her clinical duties include serving as an acute stroke telemedicine provider and leading a multidisciplinary cardiovascular risk clinic focused on hypertension control and self-measured blood pressure at the Louisville Federally Qualified Health Centers.

In 1986, Jane graduated from Catholic University, Washington DC with a Bachelor of Science in Nursing (BSN), and in 1991, she earned a Masters of Science in Nursing (MSN) from the University of Kentucky as a Clinical Nurse Specialist (CNS). In addition to her work at Baptist Health Lexington, University of Kentucky, and Washington Hospital Center, she has also served as an intermittent stroke reviewer for The Joint Commission. In the span of her career, Jane has been responsible for various clinical areas including PACU, Surgical/Trauma ICU, Cardiovascular Recovery Room, Heart Transplant, LVADs, as well as being the Nursing Representative to the Ethics Committee and Institutional Review Board.
As the Stroke Program Coordinator at Baptist Health Lexington since 2016, Jane assisted the stroke team in achieving initial Comprehensive Stroke Certification. Baptist Health Lexington is one of the highest-awarded stroke programs in Kentucky according to the American Heart Association’s Get With the Guidelines Stroke registry. In addition to serving on the Kentucky Heart Disease and Stroke Prevention Task Force Steering Committee, Jane is excited to co-chair the Kentucky Stroke Quality Improvement Project (SEQIP). Currently, she is also the chair of the statewide Door In Door Out subcommittee, to improve transfer times for stroke patients to higher levels of care.
Jane is a member of AANN and ANVC.
